You can cancel your account by going to http://cancel.midphase.com You would need your billing username and password to fill out the cancellation form you can get that by going to https://secure.mpcustomer.com and clicking on the "forgot password" link.

Please contact the billing department at 1-866 Midphase Monday through Friday 9-5 central time if you need any further assistance with your cancellation or if you have any questions.

You will be notified once the cancellation has been processed.
